I'm sure they will ask for your next of kin, but you can mention someone closer to you (location wise)as a personal choice because of that fact alone PLUS the close relationship you have PLUS the fact about the living will...which they will also ask about. Sometimes you can discuss this with your personal doctor or the doctor responsible for the surgery as far as getting authorization (doctor's orders) to let Gino (or whoever you want) to be the "family" member, responsible party, contact person, etc. Unless you feel someone might actually contest it, you can go as far as to get a legal order...something that designates Gino (or whoever) you want as the recognized person to be there and to be responsible (sort of like a guardian) while you are hospitalized and/or recovering. (Go for the clearance first, including doctor's orders if you have to or want to. Then, go for legal "stuff" if you want to or need to.)
(((Hugs))) to you Gino. I've been wanting to do that and say hi for some time now. We all want nothing but the best for Martin because we care for him so darn much! He has been a good friend to me although we've never met in person...better than some people I've known all my life. You're a dear friend of his and he listens to you, so you are his best medicine right now. Please, keep us informed and remind him that I and his friends are all here and with him all the way.
Consider the weight a temporary fullness, consider the tubes as (oh, I can't say it ), consider all the attachments as extended jewelry (bling to the max), and consider the scar as a future lacey pattern to be admired by interested "parties".

Ah, yes! Make that list because you are good at making lists. And, as you keep thinking up questions to add to the list, you'll have more things to actually think about than wonder about. When you finally get to sit down with your doctor, you can pull out your list and deal with all you've been wondering about.
